What Is Continuum of Care in Treatment?

Your needs during treatment are different from other people’s needs. For that reason, it is important that you receive the right level of care. The definition of continuum of care revolves around that aspect.

Basically, it is an extensive treatment plan that ensures that you receive sufficient care from beginning to end. It guides you through recovery and tracks your progress. When a treatment method does not work, the continuum of care can transition you into a method that does.

Elements of an Extensive Care Plan

Early Intervention and Detox

The withdrawal that occurs could be painful and trigger life-threatening symptoms. However, medical detox makes sure that you get through withdrawal safely and as comfortable as possible.

Addiction Rehab

Most of the time, treatment continues with rehab after detox. How much time that you spend in addiction therapy every day will depend on the intensity of your dependency. You might have individual sessions every day or only twice a week. In between these sessions, you could participate in group counseling and experiential therapies.

Outpatient Rehab and Aftercare

Addiction treatment services do not stop with rehab care. The reason is that moving from a drug-free environment to a normal one can cause a lot of stress. With outpatient rehab and aftercare services, the transition is smoother. They provide ongoing support to prevent relapse.

Outpatient rehab can involve different levels of care depending on your needs. A standard program only includes two or three days of treatment a week. An intensive program can consist of three to five days, and a partial hospitalization program usually includes five or six days.

Aftercare is a little different. Sometimes it includes a sober living home, where you live in a house separate from the rehab facility. It gives you time to adjust to living on your own but continues to provide therapy and supervision. Other aftercare services are private therapy and support groups.
